Barriers Associated With Treatment Nonadherence in Mexican Patients With Ulcerative Colitis: The Role of Medication Beliefs

Abstract
Background: The adherence to medical treatment is important in patients with inflammatory bowel disease (IBD) in order to maintain remission and avoid complications. The aim of this study was to identify the demographics, clinical features, and patients’ barriers associated with nonadherence to medical treatment in Mexican patients with ulcerative colitis (UC). Methods: This cross-sectional and comparative study included 121 patients with confirmed diagnosis of UC between August 2017 and January 2018. Patients’ barriers were assessed using the specific section of barriers of the Beliefs about Medicines Questionnaire (BMQ), and the Self-Efficacy for Appropriate Medication Use Scale (SEAMS) was used to assess treatment adherence. Results: According to the BMQ barriers, 31.4% (n = 38) of the participants reported negative beliefs about treatment, 55.4% (n = 67) recall barrier, and 46.3% (n = 56) access barrier to medication. The mean score of the SEAMS was 30.0 points (S.D. = 7.0), and according to the cutoff point, we found that 50.4% (n = 61) of the participants were nonadherent to their current treatment. Recall barrier (OR = 7.6), negative beliefs about medication (OR = 6.6), and disease severity (OR = 5.9) were factors associated with medication nonadherence. Conclusions: The frequency of medical treatment nonadherence was 50.4% in Mexican patients with UC, and the barriers associated were recall barrier, negative beliefs about medication, and disease severity. ©The authors ©Wiley.
