    Open surgery while wearing night vision goggles

    Night vision technology is nothing new. In fact, the military rely significantly on this technology during nighttime operations. A surgeon is like a medical soldier in the battlefield. His/her only mission is that of keeping people alive. Due to many technological advances, patients cannot only train on relaxation while visiting their doctors but also get distracted by playing videogames while waiting for them. Furthermore, this virtual reality experience can be enhanced if the patient wears goggles or Head Mounted Displays under dimmed or absence of lights. The purpose of this study was to test if a surgeon could operate when extending into such a dark condition situation, but in the surgical suite. Therefore, a surgeon performed seven open surgeries on rabbits. All surgeries were performed on the thorax and abdomen regions. Specifically, the surgeon was able to perform these surgeries by wearing on his head a micro camera with infrared light and a night vision goggles. The first assistant used this same system while the scrub nurse and the anesthesiologist did not. There were no complications either during or after these procedures. It is possible to make open surgeries wearing a night vision system. Further approaches should be tested with human volunteers. Keywords: Night Vision Goggles, Open Surgery, Cyber-medicine

    A CRAC-like motif in BAX sequence: Relationship with protein insertion and pore activity in liposomes

    Several proteins that interact with cholesterol have a highly conserved sequence, corresponding to the cholesterol recognition/interaction amino acid consensus. Since cholesterol has been proposed to modulate both oligomerization and insertion of the pro-apoptotic protein BAX, we investigated the existence of such a motif in the BAX sequence. Residues 113 to 119 of the recombinant BAX α5-helix, LFYFASK, correspond with the sequence motif described for the consensus pattern, -L/V-(X)(1-5)-Y-(X)(1-5)-R/K. Functional characterization of the point mutations, K119A, Y115F, and L113A in BAX, was performed in liposomes supplemented with cholesterol, comparing binding, integration, and pore forming activities. Our results show that the mutations Y115F and L113A changed the cholesterol-dependent insertion observed in the wild type protein. In addition, substitutions in the BAX sequence modified the concentration dependency of carboxyfluorescein release in liposomes, although neither pore activity of the wild type or of any of the mutants significantly increased in cholesterol-enriched liposomes. Thus, while it is likely that the putative CRAC motif in BAX accounts for its enhanced insertion in cholesterol-enriched liposomes; the pore forming properties of BAX did not depend on cholesterol content in the membranes, albeit those mutations changed the pore channeling activity of the protein. Su importancia radica en que se ha actualizado en cada reedición y ha cubierto los diferentes campos de la buena práctica clínica. Por ello, en esta cuarta edición, los editores nos hemos propuesto a volver a presentar el libro modificando el formato para facilitar su lectura, incrementando temas y seleccionando a los autores para que sean verdaderos expertos en Bioética y cuenten con educación formal en esta especialidad. Un elemento muy importante de esta edición, respecto a las anteriores, es la unificación del aparato crítico, si bien cuando los autores son filósofos o abogados, la referenciación bibliográfica no sigue la normativa Vancouver, original o modificada, como el resto de los capítulos.     Se llevó a cabo una encuesta a los encargados de establecimientos privados que realizan diagnóstico molecular predictivo en la Ciudad de México encontrándose que, desde la perspectiva bioética, estos estudios se enmarcan principalmente en el conflicto de intereses entre beneficencia y autonomía. Se analizó información sobre las pruebas genéticas que se ofrecen a los usuarios a fin de determinar las implicaciones éticas que esto conlleva y finalmente, poder establecer una base para una guía de práctica clínica, ya que en México no existe, prácticamente, ninguna regulación al respecto.

    The contribution of self-concept to sleep quality in adolescents: A cross-sectional study

    Self-concept has been associated with health-related behaviors and emotional self-regulation, which can improve sleep. However, its involvement in sleep quality in a healthy adolescent population has been little studied. This study evaluated the association between self-concept and sleep quality in adolescents adjusting for sleep/wake habits. The cross-sectional study included 1,751 adolescents, 54% females, with an age of M = 16.76 ± 1.04 years. The information was collected through an electronic survey that contained the Patient Health Questionnaire, Sleep Habits Questionnaire, Pittsburgh Sleep Quality Index, and Self-Concept Scale. A significant association between self-concept, adjustment for adolescents´ characteristics and sleep/wake habits was observed (R2 = .17, F = 26.61, p < .001). That is, in addition to the adolescent habits, self-concept also contributed to the explanation of sleep/wake habits. These results reinforce the idea that the self-concept can be an essential factor that contributes to better sleep quality, despite the habits of adolescents.

    Sarcoma sinovial pobremente diferenciado de pared del tórax con características rabdoides

    We report a rare case of a poorly differentiated synovial sarcoma (SS) with rhabdoid features. A 33-year-old woman was referred to our hospital with a chest wall tumor. MRI revealed a diffuse mass that invaded the pleura and extended into the esophagus, aorta, diaphragm and pancreas. Histopathological examination of the neoplasm showed it to be composed of sheets of small/medium cells with rhabdoid morphology, consisting of round, eccentrically localized nuclei, conspicuous nucleoli, and eosinophilic cytoplasm. Immunohistochemical studies demonstrated the tumor cells to be positive for TLE1, Bcl-2, EMA, CAM5.2, CD138 and CD56 and negative for desmin, smooth muscle actin or S100 protein. Fluorescent in-situ hybridization technique, performed on the paraffin section, showed SS18 gene rearrangement in the nuclei of the tumor cells. Poorly differentiated SS with “rhabdoid” features was diagnosed. This is only the 8 th case of a SS with “rhabdoid” features reported to date.

    Tumor adenomatoide del aparato genital. Estudio clinicopatológico e inmunohistoquímico de 9 casos

    Objetive: Describe the histological and immunohistochemical features of nine genital tract adenomatoid tumors . Material and methods: Nine cases of adenomatoid tumors were collected from the files of the Pathology department at a private hospital (ABC Hospital). Tumors were studied from a histological and inmunohistochemical perspective. Results: Eight women and one man were studied. Age range was 28–54 yrs. Tumors were located in the uterus (seven), fallopian tube (one) and epididymis (one). Tumor size ranged from 0.4 to 5.8 cm. We observed three histological patterns: adenoid, angiomatoid and solid. Arrangement of the neoplastic tubules around fascicles of smooth muscle; angiomatoid pattern with a peripheral location, and solid and adenoid patterns with a central location in the tumor were some of the observed histological features. Immunohistochemically all tumors exhibited strong and diffuse positivity for calretinin and AE1/AE3. Thrombomodulin was positive in all tumors (focal and weak in angiomatoid pattern and diffuse and strong in adenoid and solid patterns). The CK5/6 antibody was positive in seven tumors (diffuse in three and focal in four). Two tumors were negative for this marker. All tumors were negative for CD31.©Gaceta Médica de México

    Genetic and environmental risk factors for chronic kidney disease

    In order to change the current state of chronic kidney disease knowledge and therapeutics, a fundamental improvement in the understanding of genetic and environmental causes of chronic kidney disease is essential. This article first provides an overview of the existing knowledge gaps in our understanding of the genetic and environmental causes of chronic kidney disease, as well as their interactions. The second part of the article formulates goals that should be achieved in order to close these gaps, along with suggested timelines and stakeholders that are to be involved. A better understanding of genetic and environmental factors and their interactions that influence kidney function in healthy and diseased conditions can provide novel insights into renal physiology and pathophysiology and result in the identification of novel therapeutic or preventive targets to tackle the global public health care problem of chronic kidney disease. © Kidney International Supplements.

    Una aproximación bioética al término enhancement postulado por el transhumanismo

    La hipótesis de este artículo es que la propuesta transhumanista de “enhancement” humano aporta a la concepción de mejoramiento humano en un sentido cuantitativo mas no necesariamente en uno cualitativo. En la primera sección caracterizaremos los dos significados más habituales del término “enhancement” utilizado por los transhumanistas: como sinónimo de aumento y como sinónimo de mejoramiento o perfeccionamiento y cómo cada uno implica una concepción subyacente de cantidad y cualidad. En la segunda sección analizaremos si una maximización de ciertas funciones o facultades implica necesariamente una mejora y de qué tipo. En la tercera sección discurriremos brevemente sobre el problema de la especificación del bien y la finalidad del ser humano, implícitos en la concepción valorativa o cualitativa del mejoramiento humano.     Longitudinal Analysis of Participants in The KEEP Mexico's Chronic Kidney Disease Screening Program

    Background and aims: The Kidney Early Evaluation Program (KEEP) is a free screening and educational program aimed at detecting chronic kidney disease (CKD) among adult individuals who are at high-risk (those with diabetes, hypertension, or family history of these conditions or CKD). Confirmation of CKD diagnosis requires persistence of albuminuria or estimated GFR <60 mL/min for at least 3 months. We undertook this study to determine in a follow-up KEEP done at least 1 year after a baseline KEEP the following: 1) CKD incidence among individuals who initially tested negative for CKD, 2) transitions between CKD stages among individuals who initially tested positive for CKD. Methods: A random sample of KEEP participants was invited to participate in a follow-up KEEP between 2008 and 2010.
